REQUEST TO PLAY AT A STUDIO PERFORMANCE NIGHT

SUBMITTING THIS FORM MEANS YOU HAVE READ AND UNDERSTAND ALL POLICIES

** Minimum attendance requirement per band is 25. Please read details below before submitting this form. Time Slots are usually assigned from 7:00 pm to 11:30 pm. In completing & submitting this form you agree to play anywhere within that time period. No time slot requests will be accepted. Thank you.

ALL BAND MEMBERS MUST BE STUDIO CLUB MEMBERS: If any band member is not a Studio Club Member at the time this form is submitted, or their Club Membership is due to expire some time between when this form is submitted and the date of the Performance Night, the band will not be considered for the line-up until the situation is resolved.

EQUIPMENT: Equipment will be provided for you to play during your set. We will provide a list of the equipment. The set-up will be the same for all bands. We cannot remove this gear from the stage for your set. Any extra equipment you bring (keyboards, amps, etc.) must be cleared directly with Denise one week before the show. Please do not leave messages with The Studio staff regarding special requests. It is your responsibility to call the day of the show to confirm the equipment requested is packed and ready to go.

CANCELLATIONS:Once the form is submitted and you receive an email confirming the submission, a $150.00 fee will be charged to the canceling band through the contact name for any cancellations, as we will have to find a replacement. All cancellations must be in writing via email and sent to music@thestudio.org. Cancellations are NOT official until you get a response via email that confirms your cancellation has been received. Do not call The Studio or leave information with Studio Staff.

ATTENDANCE: The minimum number of people required per band is 25. A tally is taken at the door to track how many customers each band brings. Please understand we have an attendance obligation for this venue & it directly affects our ability to rebook this venue on a continuing basis. Therefore, everyone in the band needs to take an active role in promoting the show. We provide the band resources, the venue & all of the gear. Each band must provide the guests. THERE ARE NO GUEST LISTS FOR THESE EVENTS.
